0 A Watershed Year What 2013 Holds for the Patient Centered Medical Home PCMH

The savvy Jeff Levin-Scherz, who blogs over at Managing Healthcare Costs has responded to the Disease Management Care Blogs snarky "Prattling Pinheads of Pessimism" post on the topic of the Patient Centered Medical Home (PCMH).

Hes not a nattering nabob of negativity or a prattling pessimistic pinhead, says he.  Hed like to be thought of as a skeptic seeking substantiation.   The DMCB wholeheartedly approves of the agreeable alliterative appellation.

2013 may well turn out to be the watershed decision year for the PCMH:

If theres no published peer-reviewed proof that it reduces health care costs, nabobs, pinheads, skeptics and policymakers will need to decide if no evidence of an impact on costs is the same as evidence of no impact on costs.

If the answer is no, THEN well then have to decide if the traditional "X causes Y" mathematical approaches to derive proof (such as a comparison of averages using standard power calculations and/or impact on expected or observeed trend) are equal to the task in a very "statistically noisy" environment involving complicated human beings.

If that answer is no, THEN well have to decide if reasonable and informed assessments of potential cost reductions, used by countless other businesses every day in other sectors of the economy, are good enough,

If that answer is no, THEN well have to decide if there is face value to the PCMH. This involves a contrast of any patient benefit versus its incremental cost.  If the benefit is worth the cost.....

THEN we may have to decide if consumers are willing to pay for it, or if health care costs will need to be cut elsewhere to pay for it.

Stay tuned!

0 What Some Would Call Diabesity I Call Diapression

Its a blue Sunday today; a Sunday marked with gray Fall skies, transitioning into Winter blues. The trees are mostly, leafless, and its long past the time when you could get away with taking an afternoon stroll outside with just a light sweater.  Looking out into the landscape, its hard to image all this pervasively barren world will come right back to life next Spring.

Such can be the seasons of Diabetes.

There are moments when one feels invincible, unstoppable... committed more than anything in the world, and running full steam ahead.  And then there are the not so honest moments; the moments when one speaks to others, gently keeping behind the curtains the deep feelings of struggle bubbling within.

Depression, and other mental health concerns, are probably the most ignored symptoms of Diabetes.  Though, quite honestly, it is hard to call Depression a symptom when studies have shown that if you are predisposed to Diabetes, it really doesnt matter which one came first: if you had Depression first, it may very well lead to Diabetes; and if you had Diabetes first, it may very well lead to Depression (http://www.webmd.com/depression/news/20101122/new-links-seen-between-depression-and-diabetes).

Adding to the clinical predisposition for Depression is the heavy guilt burden being heaped upon patients by outsiders, and by what I would call an "Uneducated Doctor Epidemic", compounded with the "Lazy Researcher Virus," and exacerbated by the "Ratings Addicted Media Tsunami."

Seriously.

What comes of all of this is a perfect storm of judgement and derision toward people with Diabetes (indiscriminate of the type.) Not that the type should even matter; NO ONE gave themselves diabetes.  No one wants this disease.  But its easier to mock overweight people; its easier to blame them for disease, and economic burdens. Why else would they come up with such a thoughtless, hurtful, and insensitive term as "Diabesity?" Because its much easier than having to self examine the health itself of our country...  Who wants to look at pollution, pesticides, HFCS, BPAs, FDA guidelines, Big Pharma medicine side effects, and COI within health guideline organizations, when there are fat people we can accuse of giving themselves a disease.  (Never mind that 33.3% of Americans are obese, but only 8.8% of Americans are diabetic, and that includes all types.) This leaves most diabetics, frazzled (to say the least), most of the time.

Now, Depression in itself, is a poorly understood, and stigma filled illness. Ive had Depression since I was, at least, 12 years old and I cant tell you the number of times people have advised me to just look on the sunny side of life, to accept Jesus, or to just shut up and deal with it. Many people really cant tell the difference between the occasional bout of sadness, and clinical Depression... and just like with Diabetes, few medical professionals are equipped with the tender understanding required to help a patient overcome and manage Depression, or monitor a diabetic patient for symptoms of Depression. Whats worse, many people think Depression is a made up, modern era disease, in which people just lack will power... the same lack of will power that led them to be overweight (or eat poor food), and give themselves Diabetes. (Even if these statements are far, far from the truth.)

Even without clinical Depression, Diabetes, in itself, can be depressing for many reasons...

  • Its just a frightening disease, with many complications and dangers -- some immediate, and some compounded over time.
  • We have to work HARD to be healthy, without any apparent reward. Most people I know struggle, as it is, when the rewards are very evident (like weight loss, or muscle building, or training for some sporting event)... When people have to work hard at something, every day, so they can avoid unknown, random complications they CANT SEE happening to their bodies, or wont know about for many years to come... its like trying to hit a piñata in a pitch black room, with a 6 inch ruler. There arent many immediately gratifying rewards in this game... And if you do ever get that piñata, you still have to mind that candy.
  • Some folks work incredibly hard, and they STILL develop complications. Sometimes our genetics are the damning factor in whether or not we develop a bad complication, and this is apparent in folks who arent even diabetic, and develop many of the complications some diabetics might get, like neuropathy.
  • Folks who develop complications, often go their medical professionals only to be treated with judgement, and contempt -- told they werent compliant, and did this to themselves. Type 2 Diabetic patients often get little support from the medical community, are given little to no education, and little to NO tools (such as test strips to monitor their numbers), and then expected to have tight control.
  • Family and friends are often NOT understanding, and judge from the outside, looking in: they may think life changes should be easy enough (because theyre not rocket science, right? -- except, they CAN be lol) and shouldnt burden the patient... yet I cant tell you the number of diabetics (particularly women with diabetes) who end up making two sets of meals, three times a day, because the rest of their family doesnt want to change their eating habits, as well. Diabetics NEED a supportive environment, and supportive families, and CHANGE AS A WHOLE, in order to thrive.
  • It just gets old.  Get up, test your blood sugar, take pills... wait... wait... wait... test, take pills, count carbs, eat, test... exercise, test... test, take pills, count carbs, eat, test, exercise, test... test, take pills, count carbs, eat, test, exercise, test... test... take pills, count carbs, eat, test... Can you keep track of it? Can you do this every day, for the rest of your life? For once Id like to SLEEP IN, or eat without thinking so much! Can you schedule bathroom breaks in between, for the side effects? Can you schedule exercise breaks, in between work, when youre not at home? Can you plan snacks to stuff in your pockets so you wont go low at work? Remember to pack your lunches, every day, for work, and always get up early enough to eat breakfast, and always pack your own meals and drinks when you go to get-togethers because no one bothers to think of diabetics or those with health needs? Can you remember to schedule all the doctor appointments for your routine care? The eye doctor, the foot doctor, the A1C, the yearly physical, the CDE? Can you remember to have enough strips and snacks when you go outdoors? CHANGE YOUR LANCET. Can you still spare an extra 20 minutes to talk to the idiot who just said you shouldnt eat that candy in your pocket for your lows, or it will give you Diabetes? Are you tired, and annoyed yet? Because let me tell you, its not fun to be the fat gal at work, with candy in her pocket, because her Polycystic Ovarian Syndrome gives her hypoglycemia, and she unloads trucks for a living, but she has Diabetes... So everyone else needs to have a say in that. And oh, yes, all throughout your day... remember to drink enough water. :) Lots of water. heh Or youll go high from dehydration.  FUN.

I dont really think we realize just what were doing to persons with Diabetes.  We worry so much about about an obesity epidemic, but instead were creating a Depression epidemic in more ways than one. We are creating an epidemic of people who want to die, in silence, from shame, rather than talk about their Diabetes, or openly care for it.  An epidemic of bullied children, and young Diabetic girls with eating disorders. If we have enough courage to not judge someone who just got infected with the HIV virus, whether it was their fault or not, why would we not have enough courage to not judge someone who just got diagnosed with Diabetes? Diabetes kills more people than Aids and Breast Cancer COMBINED.

Diabetic patients need well rounded, overall care, and mental health services to help cope with this chronic, life long condition. Diabetics need support; not judgement.

If you are depressed today, friend... Understand that Winter doesnt last forever.  Just like your trees, plants, shrubs, and lawn, may require some tender, Winter loving care... So do you.  Be KIND to yourself. It is OKAY to feel the way you do.  I feel it, too.

It seems like the end of the world out there, in your heart and mind, but it is not.  I GET IT.  You want the roller coaster to stop...  If you are struggling today, with Depression, hiding behind society fabricated shame is not the answer; please, open up. Talk to your doctor, or find a new doctor.  Find a Diabetes support group in your area, or online... Leave unsupportive environments, and judgmental people (even if they themselves have Diabetes.) Do NOT go down into that Wintery scene...   You can do this, and Im no different than you... Spring is just around the corner. Let yourself bloom.

0 What are the benefits of eating orange

Orange strengthens your emotional body, encouraging a general feeling of joy, well-being, and cheerfulness. They are popular with athletes because they can be easily eaten for a burst of energy. Delicious and juicy orange fruit contains an impressive list of essential nutrients, vitamins, minerals for normal growth and development and overall health and well-being.

Eating oranges provides many benefits. They are rich in Vitamins C and A, flavonoids, antioxidants, calcium, magnesium, potassium, dietary fiber etc. Oranges have more than 60 flavonoids and 170 phytonutrients that have been shown to have anti-inflammatory, anti-tumour and anti-oxidant properties.
It is no wonder that they are one of the most popular fruits in the world. Oranges are generally available from winter through summer with seasonal variations depending on the variety.Botanically, orange is the citrus fruit belonging to the family of Rutaceae of the genus; Citrus and scientifically known as "Citrus sinensis."

An orange is a real powerhouse of phytochemicals which may help to prevent age related illness. Phytochemicals are plant chemicals that appear to offer no nutritional benefit for the body, but combine together to give protection to cells. This is already seen in plants and scientists believe that the same effect occurs in human cells. Oranges can be had not only as a snack but also as a major recipe ingredient in various dishes.

Here are some of the health benefits for oranges.

1) Natural source of Vitamin C

Oranges, like other citrus fruits, is an excellent source of vitamin C (provides 53.2 mg per 100 g, about 90% of DRI); Vitamin C is a powerful natural antioxidant. Consumption of foods rich in vitamin C helps the body develop resistance against infectious agents and scavenge harmful, pro-inflammatory free radicals from the blood.

2) Appetite booster
Studies have shown that the pectin present in oranges can help to suppress appetite for up to four hours after eating. This means that oranges are an excellent food for dieters.

3) Anti aging
Oranges help to activate the bodys own detoxification process and even the skin and zest of this fruit contain anti-oxidants which help to transport oxygen around the body and protect the skin from free radicals which cause signs of aging. In other words, eating oranges can help to keep the wrinkles away.

4) High in fiber
Oranges are an excellent source of dietary fiber and can be added to salads. Slices of orange can really liven up a boring salad, especially if you are watching your weight.

5) Lowers cholestrol
Oranges are full of soluble fiber, which is especially beneficial for lowering cholesterol levels. Soluble fiber attracts water in your gut, forming a slow-moving gel. As this gel substance travels through your intestinal tract, it picks up some of the excess cholesterol compounds and pushes them out through fecal waste. Your cholesterol levels will go down over time, decreasing your risk of suffering from heart disease.

6) Helps prevent cancer
Vitamin C is the primary water-soluble antioxidant in the body, disarming free radicals and preventing damage in the aqueous environment both inside and outside cells. Inside cells, a potential result of free radical damage to DNA is cancer. Especially in areas of the body where cellular turnover is especially rapid, such as the digestive system, preventing DNA mutations translates into preventing cancer. This is why a good intake of vitamin C is associated with a reduced risk of colon cancer. It also helps to fight skin, lung, breast and liver cancer.

7)Prevents Kidney Diseases
The regular intake of orange juice prevents kidney diseases. It also reduces the risk of forming calcium oxalate stones in the kidney. But take the juice in moderate amounts. Otherwise it can cause the decay of bones and teeth.

8) Boosts Heart Health
Oranges are full of potassium, an electrolyte mineral is responsible for helping the heart function well. When potassium levels get too low, you may develop an abnormal heart rhythm, known as an arrhythmia.
Oranges are a good source of vitamin B folate which helps to support the production of haemoglobin and also help keep blood pressure under check due to the presence of magnesium, thus protects the heart.

According to a study in Finland, women who ate oranges regularly had 50% less risk of heart disease compared to those who didnt eat them regularly. This is thought to be because the flavanoids in this fruit are helping to protect the cells. One orange provides more than 7% of the daily requirement of potassium which is needed to keep the fluid levels within the body in proper balance.

9) Promotes good vision
Oranges are rich in vitamin A, which includes a group of compounds that protect your eyes. Carotenoid compounds of vitamin A, like lutein, beta carotene and zeaxanthin, can help prevent age-related macular degeneration, a condition that leads to blindness.

10) Fights Against Viral Infections
Studies show that the abundance of polyphenols in oranges protects against viral infections.

11) Relieves Constipation
Oranges are full of dietary fiber which stimulates digestive juices and relieves constipation.

12) Promotes bone health
Oranges are also good for your teeth and bone. They are rich in calcium. The high level of calcium in oranges helps to keep the bones and teeth strong.

13) Good for mens
Orange is also good for men because it helps keep their sperm healthy. According to studies, there is a compound in orange called folate. Folate is said to contain nutrients good for the health of a sperm. Folate besides keeping the sperm healthy also protects it from genetic damage.

14) Provides Smart Carbs
Oranges like all fruits have simple sugars in them, but the orange has a glycemic index of 40. Anything under 55 is considered low. This means as long as you don’t eat a lot of oranges at one time, they won’t spike your blood sugar and cause problems with insulin or weight gain.

Oranges are highly beneficial for your body. A medium, 2 1/2-inch orange offers lots of vitamins, minerals and fiber, all for a measly 60 calories. While orange juice is also full of nutrients, youll miss out on fiber, so it is better to consume the whole fruit. Oranges are easy to tote around, since the thick skin protects them from bruising. Toss one in your bag for a quick mid-morning snack, add orange wedges to a grilled chicken salad or find other ways to add more oranges to your diet

0 What Is The Difference Between Pay for Performance and Accountable Care Organizations

Wheres that evidence?
The Disease Management Care Blog has occasionally wondered about the difference between pay for performance (P4P) and accountable care organizations (ACOs).  While the latter involves a more explicit transfer of risk, it thinks the underlying logic for both is ultimately the same: better quality performance should translate into less sickness, fewer specialist physician visits and reduced hospitalizations which, in turn, should save money that can be shared with the providers.  Assuming thats true, the possibility of getting that kind of money should prompt physicians to prevent, coordinate, counsel, immunize and screen instead of treat, er, ignore, not immunize and hustle out the door.

Unfortunately, a recently published Cochrane Review says otherwise.

The DMCB likes CRs because they are state-of-the-art and highly disciplined reviews of the worlds scientific medical literature.  In this instance, after an exhaustive examination of every published paper on the topic of P4P, the authors conclude....

"....there is insufficient evidence to support or not support the use of financial incentives to improve the quality of primary health care."

The DMCB will be the first to argue that academic, rigorously conducted, randomized and controlled clinical trials are not the final answer when it comes to managing the subtle complexities of physician behavior change.  The DMCB has run into some very smart and passionate physician leaders who have few doubts about the real world links between P4P, increased quality and reduced cost.

That being said, given the commonalities between P4P and ACOs, no one should assume that ACOs are going to succeed based on any notions of "evidence" that the promise of savings will translate into a changed physician workforce.   Cochrane says it just aint there.

0 What is Gluten

Gluten has recently become a topic of discussion across the globe as people attempt to get healthy, without improving and adjusting living habits and eating plans. The market for gluten-free products is exploding. Why exactly we don’t know. Many people may just perceive that a gluten-free diet is healthier for them without even knowing "what Gluten Is?".

So, what is this gluten  do you know anything about it! WTF.....no.......than, why do you think gluten free is healthy for you.... let me show you...

What the hell is this gluten?

It is a mixture of two proteins present in cereal grains, especially wheat, barely and rye etc. which is responsible for the elastic texture of dough. Most of us unknowingly love it, because gluten gives our favorite foods that special touch: It makes pizza dough stretchy, gives bread its spongy texture, and is used to thicken sauces and soups.

But, due to some health issues peoples with celiac disease should not eat products with gluten. To people with celiac disease, gluten is truly evil: Their bodies regard even a tiny crumb of it as a malicious invader and mount an immune response. This immune reaction ends up damaging the small intestine, which causes both great gastrointestinal distress and nutritional deficiencies. If untreated, these responses can then lead to intestinal cancers as well as complications such as infertility and osteoporosis.

Symptoms of celiac disease include diarrhea, anemia, bone pain, and a severe skin rash called dermatitis herpetiformis. But celiac disease often has few or no symptoms. So most of the cases still remain undetected.

How can you know if you have celiac disease? The only way is to be tested. The first test is typically a blood test that detects antibodies related to an abnormal immune response. If the blood test is positive, a biopsy is performed to confirm inflammation in the lining of the small intestines.

Some recent research studies reveal that nonceliac gluten sensitivity can lead to similar symptoms such as stomach cramps, diarrhea, and bloating. But unlike celiac, sensitivity doesnt damage the intestine. For years, health professionals didnt believe nonceliac gluten sensitivity existed, but experts are beginning to acknowledge that it may affect as many as 20 million Americans.

This is the reason why gluten free diet which is generally used for treating celiac disease works for people who may be sensitive to gluten but don’t have outright celiac disease. These people may feel better on a diet with less gluten. But that dosent mean gluten free means healthy for the rest of us. In fact, going gluten-free means saying no to many common and nutritious foods.

It is a mere health hype thanks to strong marketing campaigns that people think going gluten free can help them lose weight faster. In fact theres nothing magical about a gluten-free diet thats going to help you lose weight. However gluten-free dining can seriously limit the number of foods you can eat. With fewer choices, youre a lot less likely to overeat. this may work for some peoples. But it can backfire too, because gluten-free doesnt mean fat-free or calorie-free.

So, Should you go gluten free?

If you have symptoms for celiac disease or gluten sensitivity, than you have no choice you have to.

But if you just want give it a try, think again.........
It is not like just a low carb or low calorie diet, going gluten free means a big pain in the butt.
Gluten-free doesnt necessarily equal healthy, especially when people yank vitamin-enriched and wholegrain foods from their diets and replace them with gluten free brownies, research suggests that those who forgo gluten may be more likely to miss out on important nutrients such as iron, B vitamins, and fiber.

However if you still dont want to let go, eat real food instead of ultraprocessed packaged fare and focus on fruits, vegetables, lean protein, dairy, and gluten free grains like amaranth and quinoa, this can be a very healthy way of eating.

0 What is colon cancer

The colon is the longest part of the large intestine and the lowest part of the digestive system. The colon makes up the majority of the large intestine, approximately six feet in length. The colon is an important part of the digestive system, and as such, it has a major role in helping the body absorb nutrients, minerals, and water.

The colon also helps rid the body of waste in the form of stool through rectum. The last six inches or so of the large intestine are the rectum and the anal canal (Anus). Colon cancer is a disease in which malignant (cancer) cells form in the tissues of the colon.

Colon cancer is cancer of the large intestine (colon), the lower part of your digestive system. Rectal cancer is cancer of the last several inches of the colon. Together, theyre often erred to as colorectal cancers.

Colon cancer is quite common, being the third most common cancer in men and women in the U.S. About 150,000 people in the U.S. are diagnosed with colon cancer and colorectal cancer each year. These are the second leading causes of cancer deaths in both men and women. According to the Centers for Disease Control (CDC), 53,196 Americans died from colon cancer in 2006 (the most recent year for available data).

What Exactly is colon cancer?

Cancer means out-of-control cell growth anywhere in your body, and colon cancer forms when this uncontrolled cell growth starts in the large intestine. At first some noncancerous (benign) clumps of cells called adenomatous polyps form on the inner walls of the large intestine. If these polyps left untreated, they may grow into malignant colon cancers over time. So it is better to get them removed during colonoscopy. Colon cancer cells will invade and damage healthy tissues near the tumor causing many health complications.

After malignant cancerous tumors form, the cancerous cells may travel through the blood and lymph systems, spreading to other parts of the body. These cancer cells can grow in several places, invading and destroying other healthy tissues throughout the body. This process itself is called metastasis, which is a very serious condition that is very difficult to treat.

What are the causes of colon cancer?

No specific causes are identified for colon cancer. When the cells uncontrollably grow and do not die, it results in cancer. There are certain factors which increase the risk of colon cancer. They include:

  • Genetics -20 % of colon cancers are caused by inherited mutations in the genes
  • Polyps- Adenomas and inflammatory polyps may become cancerous.
  • African-American race. African-Americans have a greater risk of colon cancer than do people of other races.
  • Inflammatory intestinal conditions. Chronic inflammatory diseases of the colon, such as ulcerative colitis and Crohns disease, can increase your risk of colon cancer
  • Low-fiber, high-fat diet. Colon cancer and rectal cancer may be associated with a diet low in fiber and high in fat and calories. Research in this area has had mixed results. Some studies have found an increased risk of colon cancer in people who eat diets high in red meat.
  • A sedentary lifestyle. If youre inactive, youre more likely to develop colon cancer. Getting regular physical activity may reduce your risk of colon cancer.
  • Diabetes. People with diabetes and insulin resistance may have an increased risk of colon cancer.
  • Obesity. People who are obese have an increased risk of colon cancer and an increased risk of dying of colon cancer when compared with people considered normal weight.
  • Smoking. People who smoke cigarettes may have an increased risk of colon cancer.
  • Alcohol. Heavy use of alcohol may increase your risk of colon cancer.
  • Radiation therapy for cancer. Radiation therapy directed at the abdomen to treat previous cancers may increase the risk of colon cancer.
Symptoms of colon cancer?

Cancer symptoms are quite varied and depend on where the cancer is located, where it has spread, and how big the tumor is. It is common for people with colon cancer to experience no symptoms in the earliest stages of the disease. However, when the cancer grows, symptoms include:
  • A change in your bowel habits, including diarrhea or constipation or a change in the consistency of your stool
  • Rectal bleeding or blood in your stool
  • Persistent abdominal discomfort, such as cramps, gas or pain
  • A feeling that your bowel doesnt empty completely
  • Weakness or fatigue
  • Unexplained weight loss
  • Iron deficiency (Anemia)
Many people with colon cancer experience no symptoms in the early stages of the disease. When symptoms appear, theyll likely vary, depending on the cancers size and location in your large intestine.

Are you at risk?

If you notice any symptoms of colon cancer, such as blood in your stool or a persistent change in bowel habits. Talk to your doctor about when you should begin screening for colon cancer. Your doctor may recommend more frequent or earlier screening if you have family history of the disease. Where as, guidelines generally recommend colon cancer screenings begin at age 50.

If your doctor suspects you may have colon cancer, youll likely be erred to specialists who treat colon cancer. Those specialist physicians will request a complete physical exam as well as personal and family medical histories.

Tests to detect (find) and diagnose colon cancer.

Fortunately colon cancer is both preventable and highly treatable when detected early. With screening colon cancer can be detected before symptoms develop. This is when the cancer is most curable. Physicians may conduct a series of tests such as, CBC and liver function test, digital rectal exam, fecal occult blood test, sigmodoscopy, biopsy, barium enema and colonoscopy.

Diagnoses are usually made after the physician conducts a colonoscopy or a barium enema x-ray (lower G series). Only colonoscopy can see the entire colon. This is the best screening test for colon cancer.

Stages of colon cancer:
If you are diagnosed with colorectal cancer, more tests will be done to see if the cancer has spread. This is called staging. CT or MRI scans of the abdomen, pelvic area, chest, or brain may be used to stage the cancer. Sometimes, PET scans are also used.

Stages of colon cancer are:
Stage 0: Very early cancer on the innermost layer of the intestine
Stage I: Cancer is in the inner layers of the colon
Stage II: Cancer has spread through the muscle wall of the colon
Stage III: Cancer has spread to the lymph nodes
Stage IV: Cancer has spread to other organs outside the colon

Blood tests to detect tumor markers, including carcinoembryonic antigen (CEA) and CA 19-9, may help your physician follow you during and after treatment.

How is colon cancer treated?

Cancer treatment depends on the type of cancer, the stage of the cancer (how much it has spread), age, health status, and additional personal characteristics. There is no single treatment for cancer, but the most common options for colon cancer are surgery, chemotherapy, and radiation therapy. Treatments seek to remove the cancer and/or relieve painful symptoms that the cancer is causing.

Surgery
Stage 0 colon cancer may be treated by removing the cancer cells. This is done using colonoscopy. For stages I, II, and III cancer, more extensive surgery is needed to remove the part of the colon that is cancerous. This surgery is called colon resection.

Chemotherapy
Almost all patients with stage III colon cancer should receive chemotherapy after surgery for 6 - 8 months. This is called adjuvant chemotherapy. The drug 5-fluorouracil can increase the chance of a cure in certain patients.

Chemotherapy is also used to improve symptoms and prolong survival in patients with stage IV colon cancer. Irinotecan, oxaliplatin, capecitabine, and 5-fluorouracil are the three most commonly used drugs. Monoclonal antibodies, including cetuximab (Erbitux), panitumumab (Vectibix), bevacizumab (Avastin), and other drugs have been used alone or in combination with chemotherapy.

You may receive just one type, or a combination of these drugs. There is some debate as to whether patients with stage II colon cancer should receive chemotherapy after surgery. You should discuss this with your oncologist.

Radiation therapy:
Radiation therapy is sometimes used in patients with colon cancer. It is usually used in combination with chemotherapy for patients with stage III rectal cancer. For patients with stage IV disease that has spread to the liver, treatments directed at the liver can be used. This may include:

  • Burning the cancer (ablation)
  • Delivering chemotherapy or radiation directly into the liver
  • Freezing the cancer (cryotherapy)
  • Surgery
Alternative Treatments

Although there is no scientific evidence to show that alternative treatments can treat or cure colon cancer, certain therapies can improve the quality of life of cancer patients. Activities such as art, dance and music can shift focus away from the disease and the treatment process, and help reduce stress.

In addition, exercise and meditation can improve mood and appetite. Support groups are also a helpful resource for coping with colon cancer. Information on local groups can be found through organizations including the Colon Cancer Alliance, Cancer Care and the American Cancer Society.

0 What Should We Eat to Lose Weight

Hi,

Brad Pilon has revised his book "Eat Stop Eat" and since I am on his mailing list from having purchased his last version I have been given the opportunity to read his revision and provide feed back to him, if I have any, before he publishes it. What a great idea. In my former life I was a proof reader and typesetter so this is "old hat" to me and I enjoy doing it. So I am again reading his book and thinking about some of his statements.

I am again struck by this one: ...research was trying to uncover the completely backwards idea, "What should we eat to lose weight?"

Sometimes it is so simple we do not see the forest for the trees. When a person wants to lose weight they immediately look for a "diet plan," or "way to eat," which nearly always has a "diet guru" to go along with it. The guru can be a person, like a celebrity, or an organization, like Weight Watchers, etc.

In reality the only thing that is needed is an actual calorie reduction. It is not completely about that, but the main thing of impact is the reduction in calories. Some people, like me, also need to pay attention to how many carbs are eaten, but, for the most part, if you simply eat less you will lose weight.

By far the easiest way that I have found to reduce calories is with intermittent fasting. You give up a few meals a couple of times a week and voila! you have reduced your calorie intact while not really changing how or what you eat the rest of the time.

The only pit fall is if you suddenly start eating more in between fasts. That is the formula for gaining weight. Remember the Sumo Wrestler exercises, fasts every morning, and eats two 10,000 calorie meals -- every day. (http://blogspot.com/2012/06/same-dedication-different-goal.html)

Is it not eating excessive amounts of food that makes us fat? So does it not follow that not eating food for a period of time would help you to lose weight? And guess what.... weight loss is not the only result. During a 24-hour fast blood sugars improve, cholesterol improves, fat is actually used rather than stored, and you feel alert, alive, and peppy, too.

Think about animals in the wild. They hunt down some food, kill and eat. Then they go without, until they find something more to eat. The ancient human hunter/gatherer did the same thing. Find a rabbit and some berries. Have a feast. The food is gone so now they fast until they find some more food to eat which could be on the same day, but was very likely, the next day.... or even the next. Water, in this scenario, becomes really important.

In between eating, they drank water.... as they fasted. Did they not per to live near water? Did not civilizations grow up right next to a body of water?

So now. Looking at my own life, when I fast for two 24-hour periods each week, I either maintain my weight or lose a little -- I dont usually gain. If I eat veggies and protein in between fasts, my body likes it better.

I started my fast today at 11:30 after eating a breakfast of steamed broccoli and lemon slices with two eggs over easy. It is always easier to start a fast after eating a nice healthy meal.

Ill go swimming in a little while, and then tomorrow at 11:30, Ill eat again.

If I do that a couple of times a week. I am in like flint.

The hard part for me is sometimes I just dont want to. I get lazy and want to do things my way (which is the way that got me into trouble in the first place). Reading Brads book always gets me motivated again because it reminds me of all the benefits of fasting.

Seeking the Lord is the same, in that if I remember the benefits, it is so much easier, and becomes more constant in my life. It is the constancy that I want.

Psalm 103:2 Bless the LORD, O my soul, and forget not all his benefits:

Read the rest of Psalm 103 and you will be blessed.

Be back soon,

Marcia

0 What Is Heart Disease

Bring up heart disease, and most people think of a heart attack. But there are many conditions that can undermine the hearts ability to do its job. These include coronary artery disease, cardiomyopathy, arrhythmia, and heart failure. Keep reading to find out what these disorders do to the body and how to recognize the warning signs.

What Is a Heart Attack?

Every year, more than 1 million Americans have a heart attack – a sudden interruption in the hearts blood supply. This happens when there is a blockage in the coronary arteries, the vessels that carry blood to the heart muscle. When blood flow is blocked, heart muscle can be damaged very quickly and die. Prompt emergency treatments have reduced the number of deaths from heart attacks in recent years.

Heart Attack Symptoms

A heart attack is an emergency even when symptoms are mild. Warning signs include:

    * Pain or pressure in the chest.
    * Discomfort spreading to the back, jaw, throat, or arm.
    * Nausea, indigestion, or heartburn.
    * Weakness, anxiety, or shortness of breath.
    * Rapid or irregular heartbeats.

Heart Attack Symptoms in Women

Women dont always feel chest pain with a heart attack. Women are more likely than men to have heartburn, loss of appetite, tiredness or weakness, coughing, and heart flutters. These symptoms should not be ignored. The longer you postpone treatment, the more damage the heart may sustain.

Signs of Coronary Artery Disease

A precursor to a heart attack, coronary artery disease or CAD occurs when  sticky plaque builds up inside the coronary arteries. This narrows the arteries, making it more difficult for blood to flow through. Many people dont know they have CAD until a heart attack strikes. But there are warning signs, such as recurring chest pain caused by the restricted blood flow. This pain is known as angina.

Inside a Heart Attack

The plaque deposited in your arteries is hard on the outside and soft and mushy on the inside. Sometimes the hard outer shell cracks. When this happens, a blood clot forms around the plaque. If the clot completely blocks the artery, it cuts off the blood supply to a portion of the heart. Without immediate treatment, that part of the heart muscle could be damaged or destroyed.

Dont Wait to Be Sure

The best time to treat a heart attack is as soon as symptoms begin. Waiting to be sure can result in permanent heart damage or even death. If you think you may be having a heart attack, call 911. And dont try driving yourself to the hospital. When you call 911, the EMS staff can start emergency care as soon as they reach you.

Sudden Cardiac Death

Sudden cardiac death (SCD) accounts for half of all heart disease deaths in the U.S., but its not the same as a heart attack. SCD occurs when the hearts electrical system goes haywire, causing it to beat irregularly and dangerously fast. The hearts pumping chambers may quiver instead of pumping blood out to the body. Without CPR and restoration of a regular heart rhythm, death can occur in minutes.

Arrhythmia: Erratic Heart Beat

Regular electrical impulses cause your heart to beat. But sometimes those impulses become erratic. The heart may race, slow down, or quiver. Arrhythmias are often harmless variations in rhythm that pass quickly. But some types make your heart less effective at pumping blood, and that can take a serious toll on the body. Let your doctor know if youve noticed your heart beating abnormally.

Cardiomyopathy

Cardiomyopathy is a disease involving changes in the heart muscle. These changes may interfere with the heart’s ability to pump effectively, which can lead to a chronic condition called heart failure. Cardiomyopathy is sometimes associated with other chronic conditions, such as high blood pressure or heart valve disease.

Heart Failure

Heart failure doesnt mean your heart stops working. It means the heart cant pump enough blood to meet the bodys needs. Over time, the heart gets bigger to hold more blood, it pumps faster to increase the amount of blood moving out of it, and the blood vessels narrow. The  heart muscle may also weaken, reducing the blood supply even more. Most cases of heart failure are the result of coronary artery disease and heart attacks.

Congenital Heart Defect

A congenital heart defect is one thats present at birth. The problem could be a leaky heart valve, malformations in the walls that separate the heart chambers, or other heart problems. Some defects are not found until a person becomes an adult. Some need no treatment. Others require medicine or surgery. People with congenital heart defects may have a higher risk of developing complications such as arrhythmias, heart failure, and heart valve

Testing: Holter Monitor

A Holter monitor is a portable heart rhythm recorder. If your doctor suspects a heart rhythm problem, she may ask you to wear one for 1 or 2 days. It  records the hearts continuous electrical activity day and night, compared with an EKG, which is a snapshot in time. The doctor will probably also ask you to keep a log of your activities and to note any symptoms and when they occur.

Testing: Cardiac CT

Cardiac computerized tomography (known as cardiac CT) takes detailed images of the heart and its blood vessels. A computer stacks the images to create a 3-D picture of heart. A cardiac CT can be used to look for plaque or calcium buildup in the coronary arteries, heart valve problems, and other types of heart disease.

Smoking and Your Heart

If you smoke, your risk of heart disease is 2 to 4 times greater than a nonsmokers. And if you smoke around loved ones, youre increasing their risk with secondhand smoke. Each year in the U.S., more than 135,000 people die from smoking-related heart disease. But its never too late to quit. Within 24 hours of quitting, your heart attack risk begins to fall.

Heart Disease Prevention

The key to preventing heart disease is a healthy lifestyle. This includes a nutritious diet, at least 30 minutes of exercise most days of the week, not smoking, and controlling high blood pressure, cholesterol, and diabetes. If you drink alcohol, do so in moderation – no more than one drink a day for women, two drinks a day for men. Ask your friends and family for help in making these changes. Theyll benefit, too.

Diet and Your Heart

What you eat makes a difference. Be sure you get plenty of whole grains, vegetables, legumes, and fruits to help keep your heart healthy.  Plant oils, walnuts, other nuts, and seeds can also help improve cholesterol levels. And dont forget to eat fish at least a couple of times each week for a good source of heart-healthy protein.

0 Technocrats Running Amok What It Could Mean For America Inc

Check out The New York Times columnist David Brooks most recent article for an insightful look at how the Europeans entrusted their collective economic future to an expert technocratic class.  Once those mandarins were unleashed, their bureaucratic disdain for undereducated commoners was only exceeded by their skill at complex obfuscation.  Egads.

On the other side of the globe, Chinas state-managed businesses are making a mess of high speed rail, creating a housing bubble and are enabling an autonomous Peoples Liberation Army.  And last but not least, the Russians have taken the concept of state sponsored oligarchy to a whole new level.

Compared to the rest of the world, carrying mortgages, automobile manufacturers and solar panel plants on the U.S. balance sheet may just have to be a new way of doing business.  If thats the case, well also need our pool of cool, pedigreed, hyper-educated Masters of the Universe like Energy Secretary Mr. Chu, who can easily triangulate on policy, politics and returns on investment. Thanks to brainy virtuosos like him, Uncle Sam, Inc. will be able to roll the dice and be a venture capitalist.

And health insurance isnt immune. The virtuosos in Washington are busy setting the essential benefit, determining the best medical loss ratio and which companies can compete in exchanges.  Generics are regulated and if there is no approval, drugs wont be covered.  Even though elections have consequences and Disease Management Care Blog readers may loyally disagree from time to time, how can we turnips dare to cross swords with experts like this?

Its ironic that with this as background, our political class can claim to understand the concerns of "Occupy Wall Street."  So does the DMCB, but that doesnt mean it isnt also worrying about those elite technocrats helping Washington become Wall Street.

0 What is Asthma

Asthma is an allergic condition when the bodys abnormal reaction to a substance such as mold, pollen, animal dander, chemicals, fumes, perfumes, certain foods or drugs. Germs in the nose and throat may also, cause asthma attacks.

When asthma from childhood, usually caused by food, but when attacked in late childhood or early adolescence and adults, asthma is usually caused by pollen, dust, chemicals. And if more than 45 years, asthma is often caused by infection. Sometimes, patients with heart attacks get asthma attacks are similar, but not the actual asthma.

Attacks more often if a patient feels tired or got great emotional stress, even iklimpun changes may also trigger attacks. During intense pressure, some air pipes clogged by thick mucus. This can lead to difficulty breathing and loud voice. As a result, air bubbles spread wide and a few broken walls. In the great attack, bleeding can also occur in the air bubbles in the lungs.

Asthma may develop gradually during attacks of inflammation of the throat or start suddenly when someone gets stimulus substances. First, one has a strong feeling in the chest. This sense subside within 1-2 hours or continuously for hours or days. Finally, the attack suddenly produce thick mucus and open airways.

To keep the remained well, patients with asthma should stay calm and do not reveal too much sense as it makes them more difficult to breathe. Inhalation aerosol is very useful during an asthma attack, including they must avoid colds / flu and cold.

In another view, Normalization PENNASIA stated that asthma occurs because the lungs can not function and maximal developed for wrapping enlargement of the lungs (pleura), so that the outward expansion was blocked by the chest and back and just wrapping the lungs are pressed into the head the lungs, eventually lung development can not be charging the maximum and reduced oxygen.

Enlargement or swelling of the lungs wrapping from spasticity ribs, exactly in the major or minor chest and causing the supply of fluid in the lungs wrapping blocked. Because of this lack of fluids, touch and wrapping lungs lungs in each breath causing bruising and swelling.

Another cause is the decreased elasticity in the diaphragm that limits the chest and abdomen and the main muscles in the respiratory process. Theore, hardening in this section is really disturbing expansion to the abdomen. Sometimes, people with asthma also have a complaint on his stomach as heartburn harden and feel pain when being pressed.

For chronic sufferers, asthma can cause lematian because of spasms in the neck. Breath that often causes neck spasms, even when the asthma attack, strangling the neck into spasm and respiratory tract and breathing stop include the supply of oxygen to the brain and throughout the body also stopped. Death came after it was due to the condition.

Of some cases, asthma can not be determined in a single location, such as in the chest, but also in the shoulder or in the abdomen. Abnormalities between one location to another has a timbale-forth relationship. For example, asthma can start from abnormalities in the chest at first and then lead to relapse with abnormalities in the stomach.

The people with asthma will experience more miserable if they are on your back because of chest parallel to the abdominal organs and lungs pressing down. Hinge ribs on the back too depressed and difficult to grow, so wrapping that surrounds the lungs will be more pressure and cause great pain. For good sleep, his position sitting on a chair and head resting on the pillow that is on the table. In addition, the bowed head is still useful because it helps the lungs to expand into the stomach when the trachea stretched down and make room for the lungs to expand when focused abdominal organs down.

For the prevention of asthma, one should avoid sneezing-coughing ataubatuk in the long term, jobs are tough, haste conditions, inadequate food, cold drinks, dirty air, cold air continuously and so on.

0 It is Not so Much What is Happening It is How You Respond to It

Hi,

I have been reading a very interesting book called "12 Steps to Raw Foods, How to End Your Dependency on Cooked Food" written by Victoria Boutenko who, I am finding, is a rather amazing person with straight forward and pointed insight into the issues surrounding "going raw" from the reasons to go raw, to how to deal with temptation. She is a person full of curiosity who sought to solve a problem in her own life: the illness that was affecting herself, her husband, her son, and her daughter. She simply started looking for the solution to her immediate problem and did not give up until she found the raw food solution which changed all of their lives. Seek and ye shall find is the foundation that she operates from and she leaves no stone unturned.

I started reading this book because I found it at the library, along with a couple of others, and I was enticed by her writing, her ideas, and her story from page one. It is excellent reading. My own belief is that raw food is the most nutritious, and most healthy food we can choose to eat, but I have had such a time struggling to do it, falling off the wagon, and having to get back up again that I was feeling like I was at my wits end, wondering why others could do it but I could not. In her book, I discovered this is not unusual and I am not alone.

From Victorias extensive experience in helping others to understand the importance of eating raw she also observed that many people who came to her classes and were enormously happy with the positive health changes that were wrought in their lives by doing what she taught, still did not remain raw foodists. At one point she discovered that every single person she had taught, not long afterwards, quit doing it. She wondered why and with the same curiosity and seeking she used to solve her health problems, she did what she needed to do to find out why this was happening, and how to solve this problem, too. What she discovered is that cooked food is addictive and that if you become aware of that, it is more likely that you can nurture your skills at getting off of the addiction and getting on to new and rejuvenating health with raw food. Now she has had the experience of visiting former students who not only remained "raw" and continued to live in their new health, but because of her students others have also gone raw and improved their health, too.

That, for me, is what I am seeking: the reasons for going raw, the how to go raw, and the way to stay raw all boiled down into something that I can keep fresh and in focus so I can actually reach my goal. Why do it if you are not going to keep doing it until there is a compelling reason to do something else? And how do you deal with temptation?

It seems that some people go raw, and stay raw for a certain number of years and then make another change as is needed for their own health. Some people go for the rest of their lives as a raw foodist. It all depends on the individual. And, I think that is one of the main points that anyone needs to understand -- that I need to understand. It is about personal mission and purpose. It is also about listening and responding to your own body and its specific health needs.

She has also given me a way to begin to satisfy my bodys cravings by feeding it highly nutritious green smoothies. She explains that the body wants and needs certain nutrients and if the foods we are eating (the Standard American Diet) are not fulfilling those needs we are left continually hungry and craving. The green smoothies are raw green leafy veggies and fruit that have been broken down for the bodys use. Breaking the food down in a blender is like a supplement to chewing which makes it easier for the body to digest and actually use the fabulous nutrients to heal what needs to be healed. Once the body has been nourished for a few weeks in this manner the cravings begin to stop and then it is much easier to go raw with less problems. She said it took her and her husband about two months, but the children adjusted more quickly. The green smoothies were something I had left out. After reading this I made one this morning which was very satisfying. Who knew? In a few weeks I will know if this is actually true or not. I will keep you posted.

One of the revelations that I have gotten from reading Victorias insight is that temptation is a distraction and can only be conquered by keeping the main goal in the fore front of your mind. As I thought back on times when I was able to accomplish some thing in my life, it did come from being completely focused on what I was going to get out of doing it. As I have gotten older something has changed in me. I suppose that is simply how life is. We do not remain as we were when we were young. Life and experience change us and sometimes the change is discouragement.

As I think about it, I am glad I did not stay where I was. I like many of the changes that have taken place in me as I have matured, but there are parts of me that I just never conquered. Weight and health being not only the most obvious, but also the most frustrating and difficult for me. It is no wonder. The standard American diet I was eating is not a healing nor a weight loss diet. It is what is causing the fat, the pain and the suffering. Not until a person realizes that for themselves and makes the drastic change to consuming healthy raw foods can they ever get beyond it. You are what you eat.

My recent setback with the Xylitol has given me a new impetus. I was taken back to square one with that one, but I have started over again, already. A friend pointed out to me her view that I did not simply stay down, that I continued to begin again, and I appreciated her encouragement. When she said that I realized that yes, there was a time in my life when for some long years I had completely given up on ever losing weight or improving  my health. But after praying for God to deliver me, I did get the motivation to no longer quit. I got the belief that I can get up and try again. Like Victoria, I just needed to seek and to find the answers that I needed.

I am about to begin doing some of the written exercises she suggests with the idea in mind of zeroing in on what my main reason for doing this is. I have kept my reasons vague, to myself, out of habit, and that seems to be one of the problems. I need to get clear on why I want to do this. What will be the benefit to me? Why is it so important? If I am honest I can see that I kept my reasons vague for the purpose of doing things my own way and continuing to sample the forbidden things. I wanted to make the change, but didnt want to change anything -- if that makes any sense. It was a way of having my cake and eating it too. But I can very clearly see that that does not work. I accomplish nothing when I plan to avoid what is really needed. If you skirt the real issues you simply keep going in circles and Im really tired of doing that. Id like to get on with it.

So. Here I am, again. I kind of like this place. It is exciting to think that I can do this. I just needed the right perspective, information, and help to get going again. I am grateful to God that he answered my prayers for help with Victoria Boutenkos experience, insight, and compassion for real life situations. Here is the URL to her web site: http://www.rawfamily.com/

I want to encourage you and let you know, that I believe you can accomplish your health and weight goals, too. I am rooting for you. Thanks for rooting for me, I appreciate it.

Be back soon

0 What is a star fruit

It is also know as Carambola

The star fruit is a tropical fruit that is gaining popularity in the United States. This fruit acquired its name from the five pointed star shape when cut across the middle of the fruit. It has a waxy, golden yellow to green color skin with a complicated flavor combination that includes plums, pineapples, and lemons.

Originally from Sri Lanka and the Moluccas, and cultivated in Southeast Asia and Malaysia for several hundred years, this fruit also goes by many other names including: bilimbi, belimbing, Chinese star fruit, five-angled fruit and the star apple. Today, star fruit flourishes in south Florida and Hawaii because the fruit thrives on growing in a warm environment. Two types of star fruit are grown, tart and sweet. Tart varieties typically have narrowly spaced ribs, while sweet varieties tend to have thick, fleshy ribs. The tastes between the two are hardly distinguishable, as the tart variety still has some sweetness. This tropical fruit is readily available July through February.

Star fruits are an excellent source of vitamin C, is low fat, and naturally sodium and cholesterol free. A small whole star fruit will provide approximately 2/3 cup sliced.

Selection
Select firm, shiny skinned, even colored fruit. Star fruits will ripen at room temperature and have lightly brown edges on the ribs when it’s ripe. Avoid purchasing star fruit with brown, shriveled ribs. This delicious fruit is also available dried.

Storage

Non-ripe star fruit should be turned often, until they are yellow in color and ripe with light brown ribs. Store ripe star fruits at room temperature for two to three days or unwashed, and refrigerated, in a plastic bag for up to one week.

Preparation
Star fruits are great to eat out of hand as these tropical delights do not need to be peeled or seeded before eating. Simply wash the fruit, remove any blemished areas, cut crosswise to get the star shape, and eat!

YUP high nutrition and low calories... NATURALLY!

Fruits and vegetables are the most nutritious foods. Most people know this to some degree. However, due todays complex and over-productive farming issues, we have depleted many of our rich and nutritious soils. Some of the darker vegetables that were once rich in nutrients are now depleted of nutrition. Most people do not have the time or energy to plant their own food, so because of this, we must look further.

Usually cutting calories almost always leads to less nutrition. But this is awful, you lose your luster, your hair and skin quality, and ultimately your health. We follow these low calorie diet with adding processed supplements that in large part are not compatible in our bodies. In laymans term, your body does not break down or use many of the vitamin and mineral capsules you spend a lot of money on.
You can get your nutrition from high-quality sources... through Superfoods!
